Occupational Therapist (OT) – Travel Contract

Pay: $2,400-2,600 per week
Contract Length: 13 weeks (extension possible)
Schedule: Day shifts | 36–40 hours per week
We’re looking for a motivated and compassionate Occupational Therapist (OT) to join our Skilled Nursing Facility in Oxnard, CA. This 13-week travel assignment offers competitive weekly pay, flexible scheduling, and the opportunity to make a meaningful impact in patient care. Responsibilities- Assess and treat SNF residents to improve functional independence.
- Develop individualized OT treatment plans aligned with care goals.
- Document therapy progress and adjust plans based on measurable outcomes.
- Monitor and report productivity and outcome metrics for quarterly targets.
- Collaborate with interdisciplinary team to coordinate patient care.
- Utilize evidence-based practices to drive improved patient outcomes.
- Maintain compliance with regulatory standards and facility policies.
- Educate residents and families on home programs and safety.
- Identify and address potential barriers to recovery and participation.
- Engage in ongoing professional development to advance OT skills.
- Lead or participate in care conferences to drive patient-centered planning.
- Manage time efficiently to ensure on-site responsibilities meet targets.
- Current OT license in the state of employment with SNF experience.
- Experience in skilled nursing: assessments, treatment planning, and progress documentation.
- Ability to develop and adjust individualized OT treatment plans with measurable outcomes.
- Proficiency in documenting progress and productivity metrics for quarterly targets.
- Strong collaboration with interdisciplinary team and participation in care conferences.
- Knowledge of regulatory standards and facility policies; compliance mindset.
- Evidence-based practice adoption and ongoing professional development.
- Competence in educating residents and families on home programs and safety.
- Effective time management to balance on-site duties and targets.
