Client in CA seeking Physical Therapist: LTC/SNF for the following shift(s):

We are looking for a healthcare professional who is ready to provide exceptional patient care in this contract/travel role. Contract/travel assignments are typically 13 weeks, with potential to extend.

Responsibilities:

• Provide high‑quality physical therapy services to long‑term care patients.

• Develop and implement individualized care plans.

• Collaborate with nursing and interdisciplinary teams.

• Document all interventions and progress in the EMR.
