Our client in the Ventura area is looking to fill their RN Case Manager position!
Duties- Develop a care plan that is based on nursing diagnosis.
- Initial assessment gets completed of patient and family to determine home care needs.
- Re-evaluates patient nursing needs regularly.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Perform other tasks as necessary.
- 2+ years of RN experience
- Must have a valid driver's license
