Admitting Registered Nurse - Hospice

Livingston Memorial is looking for a Full-time Admitting Registered Nurse, 40 hours per week to support our Hospice team. Schedule is Thursday-Monday 12pm to 8:30pm.

This position will specialize in the initial evaluation and admission of patients to hospice care. They assess patients' medical conditions, explain hospice services, and guide patients and their families through the admission process. They work as part of an interdisciplinary team to create individualized care plans focused on comfort and addressing both medical and emotional needs.

• Conduct and document the nursing component of the comprehensive assessment on admission.
• Determine program eligibility, needed services, and verifies payer sources.
• Perform thorough physical and psychological assessments to determine patient needs and appropriateness for hospice care.
• Provide information about hospice care, its philosophy, and services to patients and their families.
• Guide patients and families through the admission process, including obtaining informed consent and completing necessary paperwork.
• Develop initial care plans in collaboration with the interdisciplinary team, addressing medication needs, equipment requirements, and other aspects of care.
• Address immediate concerns and provide initial symptom management as needed during the admission process.
• Ensure clinical documentation is complete, accurate, professional, and submitted to Livingston by end of day.

• Valid California Registered Nurse License.
• Current Basic Life Support Certification.
• One-year professional nursing experience.
• Bilingual in Spanish preferred.
• Valid Driver License and auto insurance.
• Knowledge of computer, proficient with Microsoft Word and Excel.
• Good verbal and written communication skills.
