Registered Nurse – Wound Care (WOCN)

- Assess, plan, implement, and evaluate wound, ostomy, and continence care for patients across multiple age groups.
- Perform comprehensive skin and wound assessments, including staging of pressure injuries, identification of infection, and evaluation of healing progress.
- Provide wound treatments such as dressings, debridement (autolytic, enzymatic, mechanical), and negative pressure wound therapy (Wound VAC).
- Manage and maintain wound VAC systems and ensure accurate documentation of therapy settings and progress.
- Deliver ostomy care and management including pre- and post-operative stoma site marking, pouching system selection, and patient education on self-care.
- Develop individualized care plans for patients with complex wounds, ulcers, and ostomies in collaboration with physicians and interdisciplinary teams.
- Educate patients, families, and healthcare staff on wound prevention, pressure ulcer prevention, and ostomy care techniques.
- Utilize EPIC for accurate and timely documentation of assessments, interventions, and outcomes.
- Monitor and evaluate patients' nutritional status and its effect on wound healing.
- Serve as a consultant and educator to nursing staff regarding best practices in wound and skin care management.
- Apply and monitor wound dressings such as alginates, hydrocolloids, hydrogels, and silver-based products.
- Identify and manage risk factors affecting skin integrity, such as immobility, incontinence, and edema.
- Collaborate with the interdisciplinary team including physicians, dietitians, and physical therapists to promote optimal patient outcomes.
- Ensure compliance with hospital policies, infection control standards, and evidence-based wound care practices.
- Participate in performance improvement initiatives related to wound and ostomy care services.
