Home Health LVN – Overnight Shift Oxnard

Due to the nature of this case, applicants may be asked to attend a pre-hire shadow as a part of the hiring process.

• Provide direct patient care in accordance with individualized care plans.
• Administer medications and treatments as prescribed by the physician.
• Monitor patients' conditions and report any changes to the healthcare team.
• Assist with daily living activities, including personal hygiene and mobility support.
• Educate patients and their families on health management and wellness strategies.
• Maintain accurate electronic documentation of patient progress and interventions.
• Participate in ongoing training and professional development opportunities.
• May be required to travel up to a 30 mile radius.

• Current LVN license in the state of California.
• Experience in Nursing
• Experience with Tracheostomies, G-tubes, and Suprapubic Catheters is preferred
• Ability to provide critical care for patients with complex needs.

• $35hr to $38hr
