Hospice RN Case Manager

We are seeking a Full Time RN Case Manager to serve patients in the Ozark, MO area. The role may also be filled on a part‑time or PRN basis if needed.

The hospice nurse spends most of the workday driving to see patients within a set geographical area. Some required meetings occur at the office on occasion. This job affords a lot of autonomy as well as great teamwork.

• Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home.
• The RN Case Manager performs head‑to‑toe assessment, provides symptom management and skilled care to patients facing terminal illnesses, and offers emotional support and education on death and dying.
• The RN Case Manager supervises LPNs and CNAs, monitors medication lists and appropriateness of medications, assesses for DME needs, and provides wound care and other treatments as needed.

• The successful candidate must be currently licensed in the State of Missouri as an RN and live within commutable distance to the office.
• Willing to train the right candidate; a gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness, work as a team player, and represent our company well in the community.
• Computer and smartphone skills for EMR charting, email, and mileage‑computation apps.
• The offices are open Monday through Friday, 8 am to 5 pm. Reliable transportation and car insurance are required.
