• Provide compassionate support by assessing emotional, social, and cultural factors related to terminal illness.
• Support caregivers by assessing their ability to cope and function in the home setting.
• Evaluate family dynamics and communication patterns while recognizing stressors that may affect care.
• Identify and connect patients and families with community and medical resources to help them remain safely at home.
• Participate in the interdisciplinary team (IDG) to develop, implement, and revise individualized plans of care.
• Offer counseling, crisis intervention, advance care planning, and assistance with end-of-life decisions.
• Ensure all care is provided in alignment with physician orders and patient/family goals.

• MSW degree from a CSWE‑accredited school of social work (required).
• 2 years of social work experience in a healthcare setting (preferred).
• Strong communication, problem‑solving, and resource‑navigation skills.
