What You'll Do:

• Develop and implement individualized MSW plans of care in collaboration with physicians and the interdisciplinary team
• Assess psychosocial needs and determine eligibility for home health services
• Provide counseling, resource coordination, and crisis intervention
• Re-evaluate and adjust care plans as patient needs change
• Support patients and families navigating complex healthcare decisions

What We're Looking For:

• Master's Degree in Social Work (required)
• Minimum one year MSW experience within the last three years
• Home health or hospice experience preferred
• Experience with geriatric population preferred
• Flexibility to meet patient scheduling needs, including new admissions
