CHFS is seeking a grant-funded Community Health Worker to provide one-on-one support through care coordination and barrier navigation, connecting clients with housing, treatment, legal aid, and employment services. The role advocates for clients, explains services, arranges transportation, and follows up to ensure attendance and progress.
The incumbent may conduct initial interviews, support recovery goals, and assist in accessing essential services in community settings.
