• Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepare and administer medications (orally, subcutaneously, through an IV) and record prescriptions. Report adverse reactions in accordance with policy.
• Record patient medical information and vital signs.
• Monitor and adjust specialized equipment used on patients, interpreting and recording electronic displays.
• Initiate corrective action when a patient shows adverse symptomatology.
• Provide bedside care for a wide variety of medical patients.
• Change dressings, insert catheters, and start IVs.
• Prepare equipment and aid the physician during patient examinations and treatments.
• Respond to life‑saving situations based on nursing standards and protocol.
• Participate in discharge planning.
• Utilize time management skills.
• Record all care information concisely, accurately, completely, and promptly in the appropriate format.
• Perform other position‑related duties as assigned, depending on setting.

• Current LPN or RN license in state of practice.
• At least one year of LPN or RN experience in a long‑term care setting is preferred.
• Knowledge of Matrix Care is preferred.
