Description:
The RN implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity.
Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit through the provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Providing patient and family-centered education and care coordination is critical. Conducting and documenting comprehensive assessments in accordance with nursing scope of practice and policies/procedures is required.
The RN considers the patient’s culture, age, and developmental level during assessments. Identifying patient risk factors and collecting health information is necessary for comprehensive care. The development of a problem list prioritizing patient needs based on input from the inter-professional team is expected. Actions and interventions relevant to patient’s culture and age must be designed. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care.
Fully implementing patient care therapies and procedures with empowerment and clinical autonomy is required within the established policies and guidelines. The RN explains procedures, treatments, and processes to the patient and family. Initiating action to alleviate patient stress and discomfort is essential. Recognizing, responding to, and communicating changes in patient condition appropriately is required. Ongoing assessment and response to procedures and therapies must be documented according to established standards and guidelines.
Delegating tasks appropriately and overseeing team members in patient care activities is necessary for effective teamwork. Monitoring the accuracy and completeness of delegated patient care provided by other team members is required. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is crucial. Facilitating the discharge process by executing discharge orders in a timely manner and assuring performance of discharge teaching is required.
