Family Nurse Practitioner | Lake Powell Region | 4-Day Week POSITION OVERVIEW

• Outpatient Family Nurse Practitioner role serving patients across the lifespan within underserved community healthcare setting

• Team-focused clinical structure supporting preventative care, chronic disease management, and long-term patient relationships
• Built-in administrative time and manageable patient volume designed to support provider balance and quality care delivery
• Scenic Northern Arizona location near Lake Powell, Antelope Canyon, and Horseshoe Bend

• Base Salary: $109,000 – $152,000
• $7,500 retention bonus beginning second year

• Four-day work week
• Monday–Thursday or Tuesday–Friday schedule options
• No call responsibilities
• Built-in administrative time

• Deliver outpatient primary care services for patients of all ages
• Manage acute and chronic medical conditions within community health environment
• Collaborate with interdisciplinary healthcare teams to coordinate patient care
• Maintain evidence-based preventative care and treatment standards
• Support underserved and culturally diverse patient populations
• Maintain clinical documentation and care coordination workflows

• Board Certified Family Nurse Practitioner (FNP-C or FNP-BC) – REQUIRED
• Arizona license or ability to obtain – REQUIRED
• Active DEA registration – REQUIRED
• Current BLS certification – REQUIRED
• Passion for underserved and culturally diverse communities – REQUIRED
• Strong interdisciplinary collaboration skills – REQUIRED
• Two years ambulatory care experience – preferred
• Navajo or Spanish language skills – preferred
