What You'll Do
  • Provide personal care for patients, including bathing, grooming, oral hygiene, and dressing.
  • Support comfort and dignity through activities such as turning, repositioning, back rubs, and fresh linens.
  • Monitor and document patient conditions, reporting changes promptly to the RN Case Manager.
  • Maintain a safe, clean, and comforting environment for each patient.
  • Collaborate with nurses, social workers, and other care team members to ensure continuity of care.
  • Accurately record care provided in accordance with company and regulatory standards.
  • Travel to homes and facilities within your assigned service area to provide compassionate support.
What You'll Bring
  • Current CNA certification in the state of Illinois.
  • CPR certification (American Red Cross or equivalent).
  • Reliable transportation, valid driver's license, and auto insurance.
  • Prior experience in hospice, palliative care, or home health preferred.
  • Compassionate nature, professional demeanor, and strong communication skills.
  • Dependable, organized, and able to work both independently and as part of a team.
