Position Overview

Inabia is seeking a dedicated Physical Therapist to join the rehabilitation team at Rehabilitation Center. This position offers the opportunity to work closely with patients recovering from injury, surgery, and chronic conditions in a skilled nursing environment.

The therapist will help patients regain independence through customized rehabilitation plans and evidence-based therapy techniques.

• Evaluate patients to determine physical limitations, strength deficits, balance issues, and mobility impairments
  .
• Design individualized rehabilitation programs aimed at improving strength, endurance, coordination, and functional mobility
  .
• Deliver therapy interventions including therapeutic exercise programs, gait training, balance training, transfer training, and manual therapy techniques
  .
• Assist patients recovering from orthopedic surgeries, fractures, strokes, neurological disorders, and general deconditioning
  .
• Track patient progress and adjust therapy plans to ensure optimal outcomes.
• Provide patient and caregiver education on exercise techniques, fall prevention strategies, and safe movement practices
  .
• Maintain detailed clinical documentation including evaluations, treatment notes, progress reports, and discharge summaries
  .
• Collaborate with interdisciplinary healthcare teams to coordinate comprehensive patient care.
• Ensure therapy services comply with facility standards, state regulations, and therapy documentation guidelines.

• Active Texas Physical Therapist License
• Graduate of accredited Physical Therapy program
• SNF or rehabilitation experience preferred

• 8-hour day shifts
