Gentiva Hospice in Texas is seeking a compassionate RN Case Manager to lead in-home hospice care. You will perform initial assessments, develop plans of care, and coordinate with physicians, social workers, and other team members to deliver individualized comfort and symptom management.
The ideal candidate is a Registered Nurse with at least 1 year of RN experience, hospice or palliative care preferred, and CPR certification.
