Transfer Access Registered Nurse RN Case Manager
The Transfer Access Registered Nurse RN Case Manager provides clinical expertise, medical communication and quality assessment for the timely facilitation and coordination of all direct admits, blood transfusions and chemotherapy across the Health First Integrated Delivery Network (IDN). The Transfer RN Case Manager will facilitate and coordinate patients transferring in or out of Health First in accordance with state and federal regulations to prevent EMTALA violations.
The Transfer Access RN Case Manager will be highly effective in critical, clinical fact gathering and analysis to identify appropriate level of care, timely acceptance and specialty care needed for transport. The Transfer Access RN Case Manager employs critical thinking skills to expedite admissions and transfers, demonstrating Health First mission, vision, values and standards in all interactions with referring facilities, physicians, employees and customers.

Minimum Qualifications:
- Education:
Associate's degree in Nursing (ASN), or Nursing Diploma. - Work Experience:
Three (3) years if clinical experience. - Licensure:
Registered Nurse (RN) licensure in the State of Florida or endorsement. - Certification:
None - Skills/Knowledge/Abilities:
Preferred Qualifications:
- Education:
Bachelor's degree in Nursing (BSN). - Work Experience:
Previous experience in Case Management. - Certification:
Case Manager Certification (CCM or ACM). - Skills/Knowledge/Abilities:
Knowledge of utilization management, and managed care reimbursement.
Physical Requirements:
- Majority of time involves sitting or standing; occasional walking, bending, and stooping.
- Long periods of computer time or at workstation.
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
