Schedule: Per Diem

Shift :

Days; 8hr: 9a-5:30p or 10hr: 8a-6:30p

• Every other weekend
• 1 major holiday and 1 minor holiday
• Must work a minimum of 4 shifts per month, 2 of those are weekend shifts

The role you ll contribute
:

The Case Management Assistant provides clerical support to the Case Managers and Social Workers to facilitate and execute discharge plans and other related activities as needed. Provides administrative support for the Case Management Department in the absence of department secretary.

The value you ll bring to the team
:

• Faxing utilization clinical reviews to third party payers
• Monitoring receipt and distribution of third-party payer requests and logs
• Assists with locating appropriate facilities for placement of patients moving to another level of care
• Facilitation of the transfer to alternate care facilities including transport arrangements
• Facilitating referrals for home care services

The expertise and experiences you ll need to succeed
:

Minimum qualifications :

• High School Grad or Equivalent
• 1 years  experience
• Previous data entry experience

Preferred qualifications :

• Good work ethic, independent worker, self-starter. Effective communicator
• Flexibility to pick up extra shifts when census increases or call-out occur
