Licensed Practical Nurse
Location:
Palm Coast, Florida, United States – Full Time.
Start Date:
Immediate.
Provides direct and individualized nursing care to assigned residents under the supervision of the Director of Resident Care (DRC). Presents a skillful mix of nursing standards, compliance with state and federal regulations, and patient-centered care.
Responsibilities- Administer medication per physician orders and document accurately on MAR.
- Document all care actions and assessment findings in a timely manner.
- Assess resident health status and report pertinent information to physicians and caregivers.
- Prepare incident/accident reports, notify physicians and families per policy, and document outcomes.
- Maintain a safe and healthy environment, following safe work methods and clinical standards.
- Assist residents in self‑care activities and education.
- Organize resources and coordinate with associates to meet residents’ needs.
- Respond promptly to emergencies and life‑threatening situations.
- Maintain confidentiality and respect resident rights regarding personal information.
- Perform additional duties requested by the supervisor.
- Demonstrate servant‑leadership, promote teamwork, and uphold CORE values.
- Attend required community meetings and participate in enrichment activities when feasible.
- Complete all required courses in accordance with HRA University.
- Graduated from an accredited LPN/LVN program and currently registered in Florida.
- Valid Continuing Education credits in accordance with state requirements.
- Minimum of one (1) year of nursing experience in a health‑care facility.
- Experience in a long‑term care community and/or geriatric nursing facility preferred.
- Experience in emergency care and first‑aid.
- Training in Alzheimer's and Dementia with a focus on Validation Therapy.
- Effective communication skills with management, associates, residents, families, and external contacts.
- Proficiency with Microsoft Office and Outlook preferred.
- Ability to make independent decisions and seek innovative practices.
- Passion for working with senior citizens.
- Background screening:
Level 2 screening through the Florida Care Provider Background Screening Clearinghouse.
