Role Description

This is a full-time, on-site Advanced Practice Registered Nurse (APRN) role based in Palm Coast, FL. The APRN will provide autonomous comprehensive patient care, including health assessments, diagnosis, treatment, and ongoing management of acute and chronic conditions within their scope of practice. Responsibilities include performing physical exams, ordering and interpreting diagnostic tests, prescribing medications, and developing evidence-based care plans in collaboration with physicians and the care team.

The APRN will deliver patient education on disease prevention, health promotion, and medication management, and will coordinate referrals and follow-up care as needed. The role also includes documenting encounters in the electronic health record, participating in quality improvement initiatives, and contributing to a culture of safety, teamwork, and patient-centered service.

Qualifications
  • Current, unrestricted Advanced Practice Registered Nurse (APRN) Autonomous license in the State of Florida and national board certification (e.g., FNP, AGNP, or relevant population focus).
  • Graduate degree from an accredited Nurse Practitioner or APRN program and active Basic Life Support (BLS) certification;
    Advanced Cardiac Life Support (ACLS) preferred based on specialty.
  • Demonstrated clinical skills in assessment, diagnosis, and management of acute and chronic conditions, including ordering and interpreting labs, imaging, and other diagnostics.
  • Experience in primary care, family medicine, or internal medicine setting; prior experience in managed care or integrated delivery systems is a plus.
  • Strong patient communication and education skills, with the ability to explain treatment plans, promote health and wellness, and support shared decision‑making with diverse patient populations.
  • Proficiency with electronic health records (EHRs), documentation standards, and basic computer applications used in clinical practice.
  • Ability to work collaboratively with physicians, nurses, care managers, and other interdisciplinary team members in a fast‑paced clinical environment.
  • Commitment to evidence‑based practice, quality improvement, regulatory compliance, and culturally responsive care.
