Licensed Practical Nurse (LPN) – Sonata Palm Coast, FL
Tuscan Gardens of Palm Coast is looking for an experienced LPN to join our team. We provide a supportive, resident‑focused environment where quality care and customer service are paramount.
Job SummaryResponsible for carrying out the instructions of the Director of Wellness. Supervise and monitor nursing staff to obtain highest quality care by performing routine nursing duties. Accountable to the Director of Wellness/Executive Director for all nursing department activities and maintaining the corporate philosophy of quality care.
Duties and Responsibilities- Supervise HHAs, CNAs and facility staff.
- Make daily rounds of all Assisted Living and retirement residents when needed.
- Observe and report symptoms and conditions of patients to the Director of Wellness or delegated RN/Physician.
- Administer medications and treatments per signed doctor’s orders with accuracy and safety, observe effectiveness and report to physician if needed.
- Inform Director of Wellness of supplies and equipment needs.
- Order medication from pharmacy when necessary.
- Supervise the Home Health Aide to ensure all nursing care is provided properly, safely, and with highest quality.
- Administer emergency first aid.
- Ensure daily care assignments for patient care and incidental duties are carried out; maintain documentation such as vital signs, weights, nurse’s notes, medication, kardex, etc.
- Maintain cleanliness and neatness of nurse’s station, medication room, and cart.
- Assist in orientation of new staff.
- Complete admission and discharge evaluations as required.
- Check equipment for cleanliness and condition.
- Provide guidance for nursing assistants.
- Promote and maintain good interpersonal relationships with residents, visitors, and personnel.
- Perform related work as required per assignment; understand duties for both assisted living and independent residents throughout the facility.
- Graduate of an accredited professional nursing program, currently licensed to practice per state.
- Work experience in the geriatric field.
- Strong communication skills, both verbal and written.
- Ability to deal with visitors, staff and residents courteously.
- Capable of lifting, pushing, pulling up to 60 lbs.
- Strong organizational skills.
- Ability to work under pressure at times.
- Maintains personal grooming, hygiene and punctuality.
- Warm, friendly, congenial personality with a professional appearance and an empathetic attitude toward seniors.
- Ability to assess resident condition and intervene when needed.
- CPR certified, or obtain certification within 90 days of hire and annually thereafter.
- May have their picture taken and image used in social media or community advertising.
- May be video recorded from devices installed by families in residents’ apartments.
