Position Overview
Advent Health is seeking a Registered Nurse (RN) to provide discharge planning and care coordination for admitted patients. The RN will participate in multidisciplinary rounds, communicate with patients and families, and develop comprehensive discharge plans.
Key Responsibilities- Participate in multidisciplinary rounds to review patient status changes, progression, and discharge planning.
- Communicate and educate patients and families about emotional, social, and financial impacts of illness; mobilize community resources.
- Assess readmitted patients to identify perceived reasons for readmission.
- Organize and facilitate patient and family care conferences with multidisciplinary teams.
- Document discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers, avoidable days, and patient/family needs per standard work.
- Communicate with payors regarding authorization for post‑acute care.
- Assess patients and families holistically for discharge planning needs across inpatient, observation, and emergency settings.
- Review medical record and incorporate clinical, social, and financial factors into the transition of care plan.
- Develop discharge plans with contingency plans, coordinate post‑acute care services and community social needs.
- Leverage technology and standard best practices to ensure continuity of care and accurate discharge reconciliation.

- Associates of Nursing (Required)
- Bachelors of Nursing (Preferred)
- 2+ years medical/hospital nursing experience (Required)
- Prior Care Management/Utilization Management experience (Preferred)
Registered Nurse (RN) – Required
Basic Life Support (BLS) – Preferred
Certified Case Manager (CCM) – Preferred
Accredited Case Manager (ACM) – Preferred
