** What You’ll Do
** As an On Call LVN, you’ll play a vital role in supporting patients during nighttime hours. You’ll work closely with RNs and physicians to ensure continuity of care and comfort for our patients.

* Key Responsibilities:

*** Follow and implement the patient’s plan of care as directed by the RN
* Provide timely and accurate documentation
* Conduct assessments and report findings to the care team
* Collaborate with interdisciplinary team members
* Use medical equipment and supplies efficiently
* Engage in ongoing professional development## ## What We’re Looking For
* Valid California LVN license
* Minimum 2 years of direct patient care experience
* At least 1 year of hospice experience preferred
* Current CPR certification

** Pay Range:** $35-$40 per hour
