** What You’ll Do
** The
** Licensed Vocational Nurse (LVN**) provides high‐quality home health nursing care under the supervision of the Registered Nurse (RN). The
** LVN
* * is responsible for delivering treatments and services in alignment with the patient’s individualized plan of care to promote safety, comfort, and optimal health outcomes.
* Implement the plan of care as developed and directed by the Registered Nurse
* Provide accurate, thorough, and timely clinical documentation consistent with the patient’s plan of care and agency standards
* Collect assessment data and perform observations, promptly reporting changes in patient condition to the supervising RN and/or physician
* Participate in the coordination of home health services by collaborating with the interdisciplinary care team to ensure seamless, patient‐centered care
* Use medical equipment, supplies, and resources safely, effectively, and efficiently
* Engage in ongoing professional development, maintaining clinical competency and participating in training or in‐service education as required
** Requirements
* ** Valid CA Licensed Vocational Nurse License
* Maintain a current CPR certification
* 2+ years of experience working in direct patient care
* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
