Licensed Vocational Nurse

Healthy Living at Home - Palm Desert, LLC is in search for a professional, knowledgeable and detail oriented ADMINISTRATIVE LICENSED VOCATIONAL NURSE (LVN) to provide administrative nursing support with our clinical leadership team providing services for our Home Health patients in the greater Temecula and Murrieta, CA areas.

Work Schedule: Please note, this is a full-time opening that included weekend scheduling. Must be available to work weekends.

The Administrative Licensed Vocational Nurse (LVN) is responsible for managing all aspects of the patient intake process to include obtaining referral information, physician orders, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

• Responsible for obtaining accurate referral information
• Obtains specific physician orders for recertification of home care services
• Ensures that physician’s orders are communicated to agency clinicians and staff with accuracy, thoroughness and understanding.
• Uses office and other patient care software
• Addresses and supports cultural practices, as along as such practices do not harm others or interfere with the planned course of medical therapy
• Participates in agency’s information management and performance improvement programs
• Complies with agency’s policies
• Assists administration in meeting the JCAHO, federal, state and location regulations and standards
• Other administrative duties as assigned

• 3 years of Home Health experience within the last 5 years
• Graduate of an accredited nursing school/program
• Current unencumbered Licensed Vocational Nurse (LVN) license
• One (1) year of experience in Home Health
• One (1) years of Licensed Vocational Nurse (LVN) experience
• Demonstrated ability to work alongside professional and administrative personnel
• Knowledge of corporate business management, governmental regulations, Joint Commission standards, and private payer practices.
• Demonstrates good communication, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

• Two (2) years or more Licensed Vocational Nurse (LVN) experience
• Two (2) years or more Home Health experience
• One (1) year taking physician orders by phone, Intake and/or scheduling/staffing

Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary. The employee must be able to lift and/or move up to 10 lbs. on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
