Position SummaryWe are seeking a Physical Therapist to provide in-home care for Medicare Part B patients. Help patients regain mobility, manage pain, and live independently - all from the comfort of their homes.
Key Responsibilities- Assess patients in their homes and create personalized treatment plans.
- Provide skilled therapy to improve mobility, strength, balance, and independence.
- Educate patients and caregivers on exercises, safety, and fall prevention.
- Document progress and ensure compliance with Medicare Part B regulations.
- Collaborate with the home health/therapy team to coordinate care.
- Adjust treatments based on patient progress and needs.
- Active Physical Therapy (PT) license in the state of practice.
- Experience in home health or outpatient therapy preferred.
- Knowledge of Medicare Part B documentation and regulations.
- Strong assessment, treatment planning, and communication skills.
- Reliable transportation and valid driver's license.
- Comfortable working independently and managing a flexible schedule.
