Physical Therapist; PT

Full time - $38-$53 an hour. Rate of pay is based on years of experience.

PRN - $53-$60 an hour. Rate of pay is based on years of experience.

• Develop Plan of Treatment for each patient based on initial and ongoing evaluations including short- and long-term goals.
• Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to residents’ progress.
• Document residents progress according to approved medical documentation and facility documentation procedures.
• Evaluate mobility, strength, and current level of resident function, while setting goals to improve in these areas that are scaled to residents’ abilities and realistically attainable.
• Report resident progress to resident, resident caregivers and loved ones, facility staff and healthcare professionals involved in ongoing treatment/care of resident.
• Assist other facility staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy.

Education:

• Graduate of School of Physical Therapy

Experience:

• Minimum of three months recent experience as a Physical Therapist working in a nursing home environment preferred.

Licenses/Certifications:

• State of Florida Physical Therapy License
• CPR Certification
