Neuro Home Health & Wound Care is looking for an experienced, motivated, and ambitious Nurse Practitioner to join our team. We specialise in Neurology and Wound Care. We are looking for someone who aligns with our core values and seeks a profession in the healthcare industry.

Order and interpret diagnostic tests, prescribe medications, develop treatment plans, and counsel patients on health and wellness.
Core duties and responsibilities- Patient assessment: Conduct thorough physical examinations, take patient histories, and perform initial patient observations.
- Diagnosis and treatment: Diagnose and treat a wide range of acute and chronic illnesses and injuries.
- Diagnostic tests: Order, administer, and interpret diagnostic tests, such as lab work and CT scans and MRIs.
- Prescriptions and therapies: Prescribe medications, therapies, and other treatments.
- Care planning: Create and implement individualized patient care plans and manage overall patient care.
- Patient education: Educate and counsel patients and their families on disease prevention, healthy lifestyle choices, and managing their conditions.
- Documentation: Maintain detailed and accurate patient records, including medical histories and test results.
- Collaboration: Work with physicians, other healthcare professionals, and specialists to coordinate patient care.
- Perform minor procedures: Carry out routine medical procedures as needed.
- Monitor patient status: Continuously monitor patient health, evaluate the effectiveness of treatments, and make necessary adjustments.
- Supervise staff: In some settings, manage and supervise other nurses and clinical staff.
- Specialized care: Provide care within a specific specialty, such as pediatrics, family health, or mental health.
Has access to all patient medical records, and patient financial accounts, which may be discussed with Clinical Manager, the Administrator, the Executive Director, the Director of Reimbursement, and the Quality Assurance Department. Has access to department's staff personnel records.
