We are seeking a Family Nurse Practitioner to join our Countryside, Florida team in a full-time outpatient primary care setting. This is an excellent opportunity for an experienced APP who is passionate about delivering high-quality, patient-centered care in a value-based care environment.
We are especially interested in candidates with primary care experience and value-based care exposure. Bilingual (Spanish) skills are preferred but not required.
Position SummaryThe Family Nurse Practitioner provides comprehensive primary care services to patients in both inpatient and outpatient settings, working closely with the Medical Director and interdisciplinary care team.
This role includes evaluation, diagnosis, treatment, and ongoing management of acute and chronic conditions, as well as a strong focus on preventive care, health maintenance, and patient education.
You will practice within established clinical guidelines while collaborating with physicians, advanced practice providers, and support staff to ensure high-quality, coordinated care.
Key Responsibilities- Obtain comprehensive health histories and perform physical examinations
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized treatment plans
- Prescribe medications in accordance with state regulations and clinical guidelines
- Order, review, and follow up on diagnostic tests (labs, imaging, EKGs, etc.)
- Ensure continuity of care through timely follow-up and documentation
- Provide patient education focused on prevention and chronic disease management
- Coordinate referrals for specialty care when needed
- Perform minor in-office procedures as appropriate
- Participate in quality improvement, care coordination, and patient education initiatives
- Collaborate with physicians, APPs, and clinical staff to support optimal patient outcomes
- Master’s degree in Nursing (APRN) from an accredited program
- Board Certified Family Nurse Practitioner (FNP-C or FNP-BC)
- Active and unrestricted Florida APRN license
- Active DEA registration
- BLS certification required (ACLS preferred)
- Minimum experience in a primary care setting preferred
- Experience in value-based care models preferred
- Strong clinical decision-making and communication skills
- Bilingual (Spanish) preferred but not required
