Suncoast Hospice, a part of Empath Health, is seeking a caring and motivated RN Case Manager in Pinellas County. The schedule includes Monday‑Friday 8a‑5p, occasional weekend days, one summer holiday, and one winter holiday. Coverage territory includes Pinellas County, working in both home and facility settings.

• Review and update the care plan, physician orders, pharmacy profile, and other records.
• Educate patients, family members, and other caregivers about the disease process and decline, available palliative interventions, the dying process, and safety practices.

• Applicants must be graduates of an approved nursing program and hold a current Florida RN license.
• Minimum one year experience in an acute‑care hospital environment providing direct patient care; prior hospice RN experience preferred.
• Valid Florida State Driver’s License (this is required only if the candidate chooses to drive themselves to a reassigned facility. It is not required if they choose an alternative mode of transportation. The cost is at the employee’s expense, except for mileage reimbursement eligibility when traveling between facilities during company time).
