, a member of Empath Health is seeking a compassionate PRN Home Health LPN to work with our Home Health team to deliver individualized care to patients in their homes in north Pinellas county
.

Locations: Home health visits in north Pinellas county (including Clearwater, Dunedin, Safety Harbor, Palm Harbor, Tarpon Springs
, and surrounding areas). If desired, visits may also be available in Pasco and Hernando counties!

Schedule
: PRN; 10-15+ home health visits per week.

• Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
• Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Perform patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

• Applicants must be a graduate of an approved nursing program.
• Minimum of 1-2 years of experience as a LPN.
• Home health care experience preferred.
• Strongly prefers experience using Homecare Homebase (HCHB) EMR.
• Intravenous (IV) Certification strongly preferred.
