- Collaborative Team:Work alongside an experienced interdisciplinary care team dedicated to exceptional resident outcomes
- Complete Start of Care assessments, admissions, recertifications, discharges, and skilled nursing visits for home health patients
- Perform comprehensive patient assessments and coordinate care plans with physicians and the interdisciplinary team
- Complete accurate OASIS documentation and maintain compliance with Medicare and home health regulations
- Monitor patient progress and identify changes in condition that require intervention or follow-up care
- Educate residents and families regarding treatment plans, medications, and disease management
- Follow all home health regulations, safety, infection control, documentation, and mandatory reporting policies
- Current Florida Registered Nurse License (required)
- Previous home health nursing experience (required)
- OASIS documentation experience and knowledge of home health regulations (required)
- Current CPR certification (required)
- Strong assessment, communication, and clinical documentation skills
- Knowledge of Medicare regulations and ACHC standards
