** Pay Range:** $55.00 - $70.00/hr
** Job Summary
** The Physical Therapist (PT) is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.
** Responsibilities
* ** Provide physical therapy services to patients according to a written physician's plan of care
* Be responsible for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver
* Provide counseling to patient, caregiver, and family
* Provide education to patient and caregiver
* Prepare clinical notes and progress summaries based on the attainment of goals
* Communicate with the physician and other health care practitioners regarding the current home health plan of care
* Initiate physical therapy program and instructs other personnel and/or family/caregiver in certain phases of physical therapy with which they may work with a patient
* Instruct personnel and/or family/caregiver as to the goals of the physical therapy program for the patient by participating in case conferences
* Train patient in the use of prosthetic device
* Identify patient and family/caregiver needs for other home health services
* Participate in discharge planning for patient
* Provide in-service education programs for nursing organization personnel
* Participate in peer consultation process
* Supervise Physical Therapy Assistants
** Requirements
* ** Active CA Physical Therapy License
* Strong verbal and written communication skills
* Strong organization skills
* Possesses and maintains current CPR certification
* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
