Registered Nurse (RN) – Home Health

Location:

Coachella Valley, Full‑Time

Pay Range: $50.00–$65.00/hr

• The Registered Nurse (RN) plans, organizes, and coordinates home health services while delivering high‑quality, patient‑centered nursing care.
• The RN uses strong clinical skills and community‑based nursing experience to support individuals and families in their homes, ensuring safe, effective, and compassionate care.
• Develop and initiate the patient’s individualized plan of care and lead ongoing, interdisciplinary assessments in collaboration with the patient, their representative, and caregivers.
• Implement appropriate preventative, therapeutic, and rehabilitative nursing interventions as ordered by the physician and outlined in the plan of care.
• Provide thorough patient and caregiver education, including disease management, medication management, safety, and home‑based care techniques.
• Identify discharge planning needs early in the care process and ensure all necessary steps and education are completed prior to discharge.
• Coordinate delivery of medical equipment, supplies, and additional services to ensure patients have what they need for safe, effective care at home.
• Supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and Home Health Aides (HHAs), ensuring care is delivered in accordance with regulatory and organizational standards.

• Valid CA Registered Nurse License
• 2+ years of nursing experience, public health nursing or home health experience preferred
• Maintains a current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
