Position: RN Manager, Care Management (SNF)

Description

The SNF CM Manager is responsible for managing member care management and coordination for members in Skilled Nursing Facilities (SNF). This individual is responsible for the leadership and oversight of assigned SNF staff and collaborating with department management and other individuals throughout the organization to support continuity of care and the delivery of quality care.

Professional Duties
  • Collaborate with Director of Care Management to ensure that departmental operations effectively and efficiently support the continuity and delivery of quality patient care.
  • Provide leadership and oversight to assigned staff. Ensure that staff have adequate resources available.
  • Monitor employee performance. Identify opportunities for improvement and/or professional development and facilitate coaching, counseling and training.
  • Ensure that employee performance evaluations for departmental staff are completed thoroughly, accurately and in a timely manner in accordance with company policy.
  • Monitor SNF Care Management metrics, such as, but not limited to: SNF Length of Stay, SNF Emergency Department (ED) Utilization, SNF to Hospital Admissions, etc. Assess departmental performance on identified metrics and progress towards achieving established goals and present findings to department Management.
  • Utilize SNF Care Management metric data to develop strategies that contribute towards the achievement of established departmental goals.
  • Provide ongoing training to departmental staff to provide updates and resources on departmental workflows, policies, procedures and workflows.
  • Participate in coordinating and leading departmental meetings.
  • Serve as a point of contact to resolve escalating concerns and situations pertaining to staff, SNF facilities, and/or patient care.
  • Establish and maintain professional relationships with staff from contracted SNF facilities, Providers, internal staff and others to promote seamless patient transitions and maintaining the SNF length of stay below 13 days.
  • Ensure that staff is regularly reviewing and evaluating each patient s plan of care to promote timely rehab progression.
  • Utilize departmental performance data and staff feedback/input to recommend process improvements.
  • Ensure that DOHC Contracted SNFs are in adherence with DOHC Contracts to include, but not limited to, Rehabilitation and Nursing services.
  • Ensure that the Specialty Referrals within the Traxit System for SNF/Custodial cases are reviewed and completed in a timely manner to ensure care is not delayed. Identify and problem solve any delays.
  • Attend Daily Hospital report as assigned to identify inappropriate/appropriate ER/Acute-to-SNF Transfers, and assist accordingly.
  • Identify Clinically complex and/or complex Psycho-Social SNF cases and assist accordingly with timely Hospice and/or Placement Transitions to ensure a timely transition of care is achieved to include utilization prevention strategies.
  • Perform other duties as assigned.
  • Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience

  • Inform patients and co-workers of how and when to contact you under normal circumstances (phone number, email, a help line, Immediate Care, etc.) or when unforeseen issues arise
Qualifications
  • Completion of an accredited nursing program. Bachelor s degree in Nursing (BSN), preferred.
  • Current California Registered Nurse (RN) License.
  • 3 years of recent acute care nursing experience.
  • Awareness of level of care, intensity of services and severity of illness, discharge screening, Skilled Nursing, and Home Health/Hospice crit ria.
  • Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
  • Detail oriented. Ability to manage time effectively, multitask and prioritize tasks to meet established deadlines.
  • Ability to utilize experience, judgement, and critical thinking skills to problem solve and manage staff.
  • Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
  • Current California driver s license and proof of auto insurance.
