Schedule: Saturday & Sundays, 6:30AM - 3PM

Position Summary

The Licensed Vocational Nurse (LVN) is an active member of the multidisciplinary treatment team and provides nursing support to clients throughout detoxification and residential treatment. Responsibilities include withdrawal and health monitoring, medication support, admissions and discharges, nursing documentation, provider and pharmacy coordination, and response to changes in client condition.

The LVN works collaboratively with BHTs, therapists, case managers, program leadership, healthcare providers, and pharmacy staff, serving as a nursing resource within their education, training, licensure, provider direction, and facility policies. The LVN is expected to remain actively engaged in the treatment environment, build professional rapport with clients, and assist with appropriate client, team, and facility needs when nursing responsibilities permit.

Essential Duties & Responsibilities

Nursing Care & Withdrawal Management

• Monitor clients' physical condition, withdrawal symptoms, response to treatment, and other health-related needs.
• Obtain and document vital signs and standardized withdrawal assessments, including CIWA-Ar and COWS, as ordered and required by facility protocol.
• Recognize changes in client condition or potential medical concerns, take appropriate action within LVN scope, and promptly communicate relevant findings to healthcare providers and the treatment team.
• Implement and follow healthcare provider orders and provide appropriate nursing education and support regarding medications, health concerns, and provider instructions.
• Respond appropriately to urgent and emergent medical situations, including initiating emergency procedures and coordinating higher levels of care when indicated.

Admissions, Intakes & Discharges

• Participate in client admissions and discharges, including applicable nursing assessments, vital signs, withdrawal assessments, medication review/reconciliation, health information review, and required documentation.
• Complete and document breath alcohol concentration (BAC) testing, urine drug screening/testing, and other authorized testing according to facility procedures.
• Identify medical concerns requiring provider review and assist with medication, provider instruction, and continuity-of-care needs during admission and discharge.

Medication Management, Provider Coordination & Documentation

• Support client medication self-administration and maintain medication records in accordance with provider orders, facility policies, IMS requirements, and applicable regulations.
• Identify and address medication discrepancies, missed doses, medication concerns, and pharmacy issues according to facility protocol.
• Maintain effective working relationships and timely communication with the Medical Director, psychiatric provider, pharmacy, and other healthcare providers regarding client condition, medications, orders, and ongoing healthcare needs.
• Participate in provider visits and consultations as requested, accurately document relevant orders, recommendations, and follow-up needs, and ensure appropriate communication and follow-through.
• Coordinate with the pharmacy regarding medication availability, refills, deliveries, discrepancies, discontinued medications, and other medication-related needs.
• Complete accurate and timely nursing documentation, including assessments, medication-related documentation, provider communications and orders, client concerns, incidents, and nursing interventions.
• Maintain appropriate medication storage, controlled medication documentation and counts, and organized and adequately stocked nursing and medication areas.

Treatment Team & Client Support

• Function as an active and engaged member of the multidisciplinary treatment team, communicating relevant nursing and health-related information to appropriate team members.
• Serve as a nursing resource when health, medication, withdrawal, or safety concerns arise, providing appropriate nursing support and guidance within LVN scope of practice.
• Work collaboratively with BHTs, therapists, case managers, and other team members, providing appropriate nursing guidance and assisting with client and facility needs when nursing responsibilities permit.
