Palmdale Regional Medical Center serves the region by providing high quality medical services to residents of the Antelope Valley, Santa Clarita Valley, and surrounding communities. The Rehabilitation Institute features 184 licensed acute care beds, inpatient and outpatient surgery, an Advanced Primary Stroke Center, cardiac services with a STEMI Receiving Center, and a 35‑bed 24‑hour emergency department. Job Duties

Reporting to the Director of Case Management, this position provides direct and indirect social services for hospital patients, their families, employees, and community members. The role works directly with case managers in daily psychosocial issue management and resolution, performs concurrent medical record reviews for post‑hospital planning, social service case finding, and quality and risk management, and acts as a social services resource specialist.

Responsibilities include unplanned readmission assessments, complex and routine discharge planning assessments, and collaboration with hospital staff to facilitate care through interaction with hospital departments and community services. The position ensures a continuum of care for patients and families through referrals to outside community agencies.

Qualifications
  • A working knowledge of basic services provided by various health and welfare organizations in the community.
  • Minimum 2 years of social service experience in a medical setting or child/elder abuse program preferred; two years’ experience in an acute hospital setting preferred.
  • Required education:
    Bachelor’s degree in social work, psychology, or human development;
    Master’s degree preferred.
  • Strong written and verbal communication and interpersonal skills.
  • Basic organizational and computer skills.
