TOGETHER CALIFORNIA is seeking a compassionate, organized, and dedicated Case Worker to support foster youth and transition-aged youth (TAY), with a particular focus on maintaining sibling connections, placement stability, permanency, and successful transitions to adulthood.
The Case Worker provides individualized case management, advocacy, service coordination, and ongoing support to youth and their families. This position works closely with Resource Parents, youth, families, courts, schools, behavioral health providers, social service agencies, and other community partners to ensure that each young person's emotional, developmental, educational, and independent living needs are addressed.

KEY RESPONSIBILITIES
Case Worker Responsibilities
- Manage individualized case plans that prioritize sibling unity, permanency, safety, and well-being
. - Conduct weekly home visits to monitor youth well-being, placement stability, and progress toward case plan goals.
- Advocate for appropriate services, educational needs, behavioral health supports, and sibling reunification when appropriate.
- Coordinate services and communicate effectively with DCFS, courts, schools, therapists, Resource Parents, and other service providers.
- Maintain timely and accurate case notes, assessments, reports, and other required documentation in compliance with applicable state requirements.
- Provide support, guidance, and resources to Resource and Relief Parents.
- Facilitate and participate in multidisciplinary team meetings, including Child and Family Team (CFT) meetings.
- Support Specialized Resource Parents in providing trauma-informed, culturally responsive, and individualized care to sibling groups.
- Monitor placement needs and advocate for services that promote placement stability and positive outcomes for youth.
- Maintain accurate and confidential case records in the agency's database.
TAY Specialist Responsibilities
- Develop and maintain a Transitional Independent Living Plan (TILP) in collaboration with each youth and their Child and Family Team.
- Provide individualized case management focused on education, employment, financial literacy, housing, transportation, life skills, and other areas necessary for successful transition to adulthood.
- Connect youth with Independent Living Program (ILP) services, vocational programs, mentoring resources, educational opportunities, and transitional housing as appropriate.
- Coordinate with Resource Parents, behavioral health providers, educational liaisons, and community partners to support successful transitions.
- Facilitate workshops and/or group sessions addressing transition readiness, independent living, and life skills.
- Participate in CFT meetings, placement planning, and permanency efforts while ensuring the youth's voice and choice remain central to the planning process.
- Monitor progress toward transition goals and ensure all required documentation and reports are completed accurately and timely in accordance with CDSS and CCR requirements.
- Provide crisis support, advocacy, and problem-solving assistance to youth navigating barriers to independence.
