SCHEDULE Business hours; evenings, weekends, travel, and on-call as needed

BASIC FUNCTION

The Residential Group Home Case Manager coordinates case management and day-to-day residential support for individuals living in assigned group homes. The position maintains accurate client and home records; monitors health, safety, service delivery, and regulatory compliance; supports and guides residential staff; communicates with individuals, guardians, families, care coordinators, medical providers, and state representatives; and assists with intakes, transitions, audits, licensing/inspection activities, and quality assurance.

The position may provide direct coverage during emergencies or staffing shortages and must remain group-home compliant.

ESSENTIAL DUTIES AND RESPONSIBILITIES Case Management and Individual Support
  • Serve as a primary point of contact for individuals, legal representatives, guardians, family members, care coordinators, and members of the support team.
  • Process new intakes and maintain required intake, eligibility, assessment, transfer, and service documentation.
  • Prepare documentation for and participate in initial, annual, expedited, and amendment-related assessments or reviews.
  • Coordinate and document admissions, move-ins, transfers, discharges, and move-outs for assigned residential homes.
  • Schedule and complete required home visits and team meetings; document follow-up needs and monitor completion.
  • Coordinate supplies, adaptive equipment, medical products, and other approved resources requested for individuals.
  • Assist with service-related forms, timesheets, authorizations, fair hearings, and correspondence with the State of Alaska as assigned.
  • Promote individualized services, informed choice, dignity, privacy, community participation, and the least restrictive supports appropriate to each person.
Residential Operations and Staff Support
  • Monitor daily residential operations and communicate priorities, changes, and follow-up needs to residential staff and supervisors.
  • Provide guidance and training to residential staff regarding individual support needs, documentation, agency policies, emergency procedures, and assigned program requirements.
  • Review staff schedules, training records, credentials, and home assignments for completeness; promptly elevate gaps to the appropriate supervisor.
  • Review residential documentation for timeliness, accuracy, consistency, and service-plan alignment; request and track corrections when needed.
  • Provide or coordinate emergency shift coverage when required and within the employee’s training and qualifications.
  • Participate in staff meetings, transition meetings, and quality improvement activities.
Documentation, Health and Safety, and Quality Assurance
  • Maintain complete and organized electronic and paper records, including client files, resident binders, home binders, and required records in Therap or other approved systems.
  • Monitor documentation related to medication administration, medical appointments, health-care follow-up, incident reporting, service delivery, individual funds, and personal property.
  • Complete and submit critical incident reports and other mandatory reports within required timelines; immediately elevate urgent health, safety, welfare, or rights concerns.
  • Assist with record requests, internal reviews, audits, investigations, corrective action follow-up, and quality-assurance activities.
  • Protect confidential and sensitive information in accordance with HIPAA, agency policy, and applicable requirements.
  • Organize and prioritize a high volume of assignments while meeting deadlines and maintaining accurate records.
Group Home and Regulatory Compliance
  • Monitor assigned assisted living homes for compliance with licensing requirements, Medicaid waiver expectations, CMS requirements, contracts, agency policy, and applicable state and federal regulations.
