Registered Nurse (RN) Inpatient Care Coordinator

We are seeking a skilled Inpatient Care Coordinator RN to coordinate patient care, support discharge planning, and ensure effective communication across the healthcare team.

• Assess patient needs and coordinate individualized care plans.
• Coordinate care with physicians, nurses, and interdisciplinary teams.
• Assist with discharge planning and transitions of care.
• Monitor patient progress and identify barriers to care.
• Coordinate referrals, follow-ups, and healthcare services.
• Educate patients and families on care plans and available resources.
• Maintain accurate patient records and documentation.
• Promote quality, safety, and continuity of patient care.

• Graduate of an accredited Nursing program.
• Active Registered Nurse (RN) license as required by state regulations.
• Current BLS certification required.
• Strong knowledge of care coordination, discharge planning, and inpatient nursing.
• Excellent communication, organization, and critical-thinking skills.
• Previous inpatient, case management, or care coordination experience preferred.
