Full-time, Monday through Friday 8a-5p

As a Home Health LPN, you will provide skilled nursing care under the direction of a Registered Nurse and physician while supporting patients and families throughout their healthcare journey.

* Responsibilities include:

- Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care
- Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team
- Assisting patients and caregivers with education and self-care techniques
- Implementing patient safety plans and utilizing available community resources
- Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes
- Documenting clinical findings, nursing interventions, and medication updates accurately and timely
- Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits
- Providing compassionate, professional support to patients, families, physicians, and colleagues
* Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
* Current Massachusetts Licensed Practical Nurse (LPN) license
* Minimum of one (1) year of clinical nursing experience
* Valid driver's license, reliable transportation, and current auto insurance
* Ability and willingness to travel throughout the assigned territory
* Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
