Location:

Candidate should reside in California and be within 50 miles of one of our PulsePoint locations (Cerritos, Costa Mesa, LA, Palo Alto, Rancho Cordova, Sacramento, Walnut Creek, or Woodland Hills).

This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

• Manage the member’s case within the scope of licensure; supervise and direct non-RN clinicians participating in the member’s case in accordance with applicable state law and contract; develop, monitor, evaluate, and revise the member’s care plan to optimize health care across the care continuum.
• Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination, and management of member needs, including physical health, behavioral health, social services, and long term services and supports.
• Identify members for high-risk complications and coordinate care with the member and the health care team.
• Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits.
• Obtain a thorough and accurate member history to develop an individual care plan; establish short and long-term goals in collaboration with the member, caregivers, family, natural supports, and physicians; identify members that would benefit from an alternative level of care or other waiver programs.
• Develop the care plan for services for the member and ensure access to those services; assist with implementation by facilitating authorizations/referrals within benefits structure or through permissible arrangements.
• Interface with Medical Directors, Physician Advisors and/or Inter-Disciplinary Teams on the development of care management treatment plans.
• Support problem solving with providers, claims, or service issues; supervise the work of non-RN clinicians (e.g., LPN/LVN, LSW, LCSW, LMSW, and others) as assigned, including task delegation, data verification, additional assessments, and input on performance as needed.

• Requires an RN; 3-4 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience providing an equivalent background.
• Current, unrestricted RN license in applicable state(s) required.

• Travel to worksites and other locations as necessary.

For candidates working in person or virtually in the locations below, the salary range for this position is $40.52/hr to $60.78/hr.
