Part-time Registered Nurse Case Manager

Anticipated Territory:
Sacramento County

Schedule:

Every Saturday and Sunday Full Days, or Saturday/Sunday plus 3 weekdays as needed.

HHS require field visits to patients' homes.

• Conduct Start of Care (SOC) admissions and OASIS assessments.
• Perform follow‑up, evaluation visits, and home visits within the designated territory.
• Reconcile medications, provide wound care, and perform treatments and procedures.
• Monitor clients’ conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver comprehensive care.
• Initiate and administer IV medication infusions (antibiotics, diuretics) following established protocols.
• Accurately document observations, interventions, and evaluations related to patient care.

• Concierge services for administrative tasks.
• Flexible work schedules; create your own schedule and hours.
• Targeted territories—typical patient visits within a 5–15 mile radius.

• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Experience with Start‑of‑Care (SOC) assessments and OASIS (highly desired).
• High level of clinical knowledge in home‑health care (preferred).
• Experience with high‑acuity patients (plus).
• Experience working with interdisciplinary teams (desired).
• Case‑management fundamentals (desired).
• Experience with IV medication infusions (highly preferred).
• Proficiency in initiating intravenous therapy (plus).
