Job Summary And QualificationsWe are seeking a dynamic Case Management RN to join our team. We need a compassionate and reliable RN to facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. In this role you will be required to coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
You would be part of a phenomenal team that works hard to support each other while providing excellent care to our patients in a collaborative manner.
- Performs a comprehensive assessment of psychosocial and medical needs of patients
- Advocates for patients while assuming a leadership role with the interdisciplinary team
- Coordinates plan of care
- Acts as a liaison through effective and professional communications with physicians, patients, families, hospital staff, and outside agencies
- Performs utilization management reviews and communicates information to third party payors
- Facilitates patient throughput with an ongoing focus on quality and efficiency
- Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation.
- BLS-AHA Provider issued card required within 30 days of start
- 3 years of RN experience in an acute care setting is required
- Prior experience in Case Management is preferred
- BSN is preferred
- Certification in Case Management, Nursing or Utilization Review is preferred
