MDS Coordinator

Americare Senior Living is seeking an experienced MDS Coordinator to oversee the assessment and care planning process for our skilled nursing residents. In this role, you will complete and manage MDS assessments, ensure accurate documentation, and coordinate interdisciplinary care plans that support residents' safety, comfort, and quality of life. You will monitor regulatory compliance, optimize reimbursement, and mentor staff on assessment standards.

Working in our resident-centered, compassionate, team-oriented environment, you'll build strong relationships with residents, families, and colleagues while supporting person-centered, dignified senior care.

Responsibilities

• Complete, review, and submit MDS assessments accurately and on time
• Coordinate interdisciplinary care planning based on MDS data and resident needs
• Ensure compliance with federal, state, and payer regulations related to MDS and documentation
• Monitor and optimize reimbursement through accurate RAI and MDS processes
• Educate and support nursing and clinical staff on documentation and assessment standards
• Collaborate with physicians, therapists, and social services on resident care plans
• Track quality measures and outcomes, recommending improvements as needed
• Communicate with residents and families regarding care plans and changes
• Maintain organized, secure resident records and assessment documentation
• Participate in audits, surveys, and performance improvement initiatives
