- Medication Aide in Memory Care, you will support that mission by providing hands‑on care, meaningful engagement, daily support, and a calm, respectful presence for residents.

What You’ll Do

As a Personal Care Assistant
- Medication Aide

, you will provide quality personal care and living services based on each resident’s choices, needs, and service agreement. You will help residents maintain independence, dignity, comfort, and quality of life.

You will:

  • Assist residents with activities of daily living based on individual needs, preferences, and care plans.
  • Support residents with bathing, dressing, grooming, hygiene, mobility, toileting, and other personal care needs.
  • Provide medication assistance following the five rights and accurately document medication support, as credentialed.
  • Take vital signs as ordered and report out‑of‑range results to the Nurse Manager or Director of Health Services.
  • Complete monthly summaries, weights, vitals, and assessments timely and accurately.
  • Assist residents to the dining room for meals and help serve, encourage, and support residents during mealtimes.
  • Engage residents in daily activities, scheduled programming, and meaningful moments outside of scheduled activities.
  • Observe residents for changes in condition, behavior, needs, or concerns and communicate promptly with the nurse and care team.
  • Complete documentation in the electronic medical record, including DAR notes by exception.
  • Wash, put away, and complete assigned resident laundry, including incontinent laundry as needed.
  • Monitor resident supplies and notify family as needed.
  • Support a clean, safe, calm, and comfortable Memory Care environment.
  • Build positive relationships with residents, families, nurses, PCAs, and team members.

We are looking for someone who is compassionate, patient, dependable, and comfortable supporting residents in a Memory Care setting.

The right candidate will be calm, observant, respectful, and motivated to put residents first. This role is a strong fit for someone who understands that Memory Care requires consistency, kindness, emotional steadiness, and a team‑first approach.

Ideal candidates will have:
  • Experience working with older adults and providing personal care services in Assisted Living, Memory Care, Dementia care, senior living, or a similar setting preferred.
  • A compassionate, dependable, and respectful approach to resident care.
  • Strong communication skills with residents, families, nurses, and team members.
  • Ability to notice changes in resident condition, mood, behavior, or needs and report concerns promptly.
  • Strong attention to detail with care, medications, documentation, safety, and follow‑through.
