Duties
Help The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.

- Patient Aligned Care Team (PACT) include, but are not limited to:
- Provide evidence-based, gender-specific nursing care to Veterans in the Primary Care/PACT outpatient setting.
- Manage assigned patient panels, utilize clinical registries, and ensure timely access, evaluation, and coordinated care.
- Deliver health promotion, disease prevention, and chronic disease management education and coaching to patients.
- Support licensed vocational nurses and nursing assistants as appropriate to clinic operations.
- Participate in nursing education, staff orientation, competency development, and quality-improvement activities.
- Collaborate with the interdisciplinary team to improve holistic primary care services and promote optimal patient outcomes.
- Evaluate nursing practice using current evidence, provide constructive peer feedback, and contribute to an evidence-based practice culture.
- Support a safe, professional learning environment and share clinical knowledge and educational insights with colleagues.
Pay:
Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
