Heartland Family Service seeks a Triage Navigator for overnight coverage at the Crisis Stabilization and Resource Center. This role screens referrals, orients clients, and assesses immediate stabilization needs while supporting trauma-informed care in a fast-paced, team environment.
You will help clients identify coping strategies, develop stabilization plans, and connect them with resources to resolve crises in the least restrictive setting.
