We are seeking In‑Home Assessment Providers (Nurse Practitioners or Physician Assistants) to support our health plan partners by delivering high‑quality, in‑home preventive health assessments.
We offer:

Compensation & Earning Potential:
Earn up to $2,400 per week (at max volume) Competitive base rate: $120–$150 per completed assessment Progressive bonus structure:

Earn over $10,000 monthly In addition to this flexible fee‑based model, you'll have the opportunity to support meaningful health outcomes, improve access to care, and work independently with the full support of a dedicated clinical team.
What You Will Do — Essential Functions As an In‑Home Assessment Provider, you will perform comprehensive health risk assessments (HRAs) for Medicare, Medicaid, and ACA members within their homes. These assessments help identify care needs, support early intervention, and improve care coordination.
Clinical Care & Assessment Delivery Perform in‑home (and occasional virtual) health assessments, including medical history reviews, physical exams, screenings, and medication reviews. Conduct thorough evaluations of chronic and acute conditions. Utilize pre‑populated clinical templates to support efficient, accurate documentation. Submit chart notes within required time frames and address queries promptly. Maintain clear communication with clinical leadership and operations teams.
Scheduling & Member Engagement Provide a self‑managed schedule with a minimum monthly availability expectation (varies by market). Offer appointment availability across days, evenings, and weekends. Deliver empathetic, patient‑centered care in a respectful and professional manner. Educate members on whole‑person wellness and connect them to appropriate resources.
Territory All providers cover a primary territory within a 50‑minute drive of their home address.
Who You Are
Required Qualifications:
Active, unrestricted NP or PA license in the state where assessments will be performed. Board certification (ANCC, AANP, or NCCPA). New York candidates:
