Position: HIM ARCHIVE COORDINATOR HEALTH INFORMATION MANAGEMENT FULL-TIME DAY SHIFT 24691

Principal

Duties & Responsibilities
  • Pick up and reconcile all discharged encounters throughout hospital units pursuant to assigned schedule.
  • Demonstrate competency in the medical record analysis process for IP, ED, LTC, OP, and SDS encounters and assist with all patient types as needed and directed by management, including chart scanning, prepping, indexing, and quality review.
  • Use computer applications such as One Content and Paragon proficiently.
  • Prioritize work and meet productivity requirements as outlined by management.
  • Retrieve medical record documentation for patient care.
  • Place documents in the appropriate order for scanning.
  • Accurately assemble LTC records.
  • Review each record for poor original documents.
  • Apply knowledge of prepping and scanning medical records to coordinate staff and appropriate workflows.
  • Reconcile and box records being sent to our archive vendor, including accessing the archive portal to request records to be picked up or requesting records for delivery when needed for ROI and internal audits.
  • Reconcile, log, and box all medical records scanned from the previous month to be placed in our storeroom for shredding after 30 days from the last day of the previous month, including transporting the boxes to building 8‑6 and placing paper charts in tote containers at month‑end for shredding by the vendor. Ability to lift boxes of records is required.
  • Process and merge medical records reported as duplicates by reviewing information in both Paragon and One Content to ensure MRNs are merged appropriately.
  • Assist physicians or other clinical staff as needed.
  • Perform duties in accordance with established procedures, protocols, and department standards.
  • Correctly identify and classify inpatient, outpatient, and emergency room records.
  • Maintain basic knowledge of the scanning process, ensuring scanned documents are legible.
  • Reconcile charts delivered to HIM via the appropriate list.
  • Participate in departmental and organizational performance‑improvement activities.
  • Adhere to the organizational Code of Conduct and Ethics, respecting patient privacy, confidentiality, and care.
  • Maintain patient, medical record, department, and employee confidentiality at all times.
Minimum Knowledge & Skills Required
  • EMR experience.
  • Microsoft Office/computer proficiency.
  • Excellent oral and written communication skills.
  • Excellent interpersonal/customer‑service skills.
Certification, Registration, or Licensure Required
  • AHIMA certification preferred.
Salary

$40,000–$60,000 per year (commensurate with experience).
